REGISTRATION FOR HOME EDUCATED STUDENTS

The School Curriculum and Standards Authority (SCSA) NAPLAN website for home educators contains important information for parents/carers who wish their student to sit NAPLAN in a non-school location. https://k10outline.scsa.wa.edu.au/home/assessment/testing/naplan/home-educators
NAPLAN materials will be posted to the person designated as ‘parent/carer’ at the address on this form.
Supervisor training information, practice test information and test codes will be sent to the ‘Test supervisor’ email address.
On receipt of this form, an acknowledgement email will be sent to the ‘parent/carer’ email address given below. Only students who are registered for home education with the Department of Education and are age-equivalent to students studying in Years 3, 5, 7 and 9 are eligible to sit NAPLAN.
The participation declaration section must be completed. All activities listed in the declaration must be carried out at the specified time for the student to participate.
All information, including student’s date of birth, must be supplied.
The student’s name will appear on their report as provided on this form.
